Health Hearts at CHO is a specialty clinic that offers a series of 8 visits for children with overweight or obesity with medical complications. The children see a physician specialized in pediatric obesity, a dietitian, a physical activity specialist, and a psychologist. The clinic is open to children age 2-18 years old. You should fill out the standard CHO referral for and order labs to fax to CHO according to the lab requisition below. In addition, children with CCHP require authorization showing that they have previously tried or are unable to attend other services, such as dietitian visits or the county pediatric obesity group visit, Active and Healthy Families.
